I was on the scene of the crash immediately after it happened and stayed with a family that was involved. There was one man airlifted and two were dead. Those dead were changing a tire on the side of the road. Here is the helicopter landing.

Since I have gotten nowhere within the city workings and I have been told that there is nothing wrong with a Director approving funds to make needed safety repairs and instead of ensuring the repairs are made; denies approving the funds and then allowing those materials purchased to fix safety issues instead they are used to build a second non permitted non inspected non lighted structure that still stands in completed, that would not meet Osha nor Fayette or Jessamine county Building codes. Since this is the issue and the fact of allowing city employees to occupy a structure that is listed as should be condemned and GRW's letter on the matter saying it may last another year. We worked in filth for years, coughing sneezing breathing fungus, standing in water, for years!!!!!! And it seems that the process of removing those whom have violated ethic, safety and all around good management decisions and allowed employee to occupy such conditions for so long is impossible.  I am sending this to each of you as well as channel 36. Below you will find that the internal audit agrees that it is no problem to approve funds to fix safety issues and then use those materials for other non-safety issues. The risk management is equally to blame,  they came out and inspected the building each year, I was told we can't have a microwave in our office, but the water and mold that’s fine.  Oh and while knowing the condition of the electrical office, the managers chose to seek funds to expand their own administration building onsite rather than seek repairs to our office. I stated it before, any managers that will allow the Managers dogs to have a nicer building onsite than the cities own employees is unfit to lead. I have given each of you notice some up to year. Oh and the temporary  office we have also has mold and some type of leak as well. If the media needs any emails please contact me I have many that show the process that took place since Sept of last year. I have tried every effort to get this done in house starting in 2012 to get repairs made to our office and this is what it takes is pathetic.

Initial Report Submitted

I transferred to West Hickman plant in Feb 2012, shortly there after MR. Charlie Martin the division director came out to review the Electricians office. The office was not, nor ever it intended to be and office, it was/is a rotting mess, if floods each rainfall, the gfci's trip due to moisture in the walls, each spring a swarm of insects came out of the walls. Mr Martin stated in front of myself and Tom Wathen that we needed a new structure that this one was unfit. The new building was not ordered, However, Mr Charlie Martin approved the purchase of roughly 7000 dollars worth of materials to repair the structure we had our offices in. He approved the purchase in Dec20102-Jan 2013. Prior to this I had also asked several times, about the required Lock-out-tag-out written policy and locks. I addressed this and lack of other polices as well as, the above mentioned item of the structure and its hazardous conditions. I learned that the material bought for repairing our office was instead used to build another none permitted structure over a group of tanks that house hazardous equipment and chemicals, It was not engineered nor designed, it was thrown up and stands uncompleted at this time. So the funds intended and approved to repair our space were inappropriately used to build another structure that isn't safe. Last Friday I was told to move to another building (solids maintenance office) which housing a type of air filtering system, I had asked that please provide why that equipment is in the room, what gases are you trying to remove, and what method of testing do you have in place. I also asked to be provided with the engineered stamped approval of the caustic and bleach tank install on the opposite side of the wall. I asked for this because this was installed in house without any engineering and/or safety monitoring equipment for leaks etc. I also inquired as to why nearly every structure in the West Hickman Waste water plant had gas monitoring but not the building that had the air filtering system. My Plant manager is well aware of my complaints, and Mr Martin has known even prior to approving funds to repair the structure that it was unfit to house employees. I feel he is unfit to lead and should have to justify the misuse of funds intent on insuring safety for employees. The point that he ignored the required lock -out policies and supply of locks and procedures as well as annual reviews for the entire tenure as a Director show that he has not taken items such as employee safety seriously. I am tired of fighting for a safe work environment. At this point Monday the 5-4-15 I was told to move our office into the administrative building until the Division could set up a mobile type structure as an office.

Additional Information Provided

I added a video to show the conditions that the Director knowing allowed employees under his supervision work in. This a brief video, but give you the idea of how long the structure was in bad condition, and even if he denies approving repairs he still allowed the employees to work in these conditions, breath mold, fungus, bird feces, the wall full of moisture, the bottom of the walls rotted out and unsupported. This is exactly the type of conditions that show intentional disregard for the health and safety of employees. The entire management team of this division section should all be removed for the disgraceful conditions they allowed my co-workers and I to work in, and we have tried for years to get repairs and they tried for years before I came to this sight. If this isn't unethical I'm not sure what is. The plant managers dogs have a nicer city owned garage to stay in than the LFUCG employees. On May 4 2015- Charlie Martin came out to look at a site to place a mobile trailer, on 5-7-2015 the trailer was delivered and we are told that we can use the shop to work in but move into the office, it has taken me 3 years to get out of this mess, and they still think its fit to use as a work shop. I would not treat an animal the way the Director and his staff have treated us.

We have 3 small children and landlord has denied about it flooding previously before we moved in even with witnesses that has seen it and nothing has been done to stop it besides the landlord tell us to put sand bags in front of the door but it comes up through the floor before the door and even through walls.
